Parent and Educator Resource Guide to Section 504 in Public Elementary and Secondary Schools
The Individualized Education Program (IEP) is a written statement of our plan to provide your child with a Free and Appropriate Public Education (FAPE) in their Least Restrictive Environment (LRE).
How do you get assistance for Early Childhood kids?
If your child has a chronic or temporary medical or mental health condition that prevents them from attending school for a minimum of five consecutive school days, the Home/Hospital Program may be able to help. The Home/Hospital Program provides curriculum instruction for qualifying students in their home or hospital room in order to provide continuity of instruction during a medical or mental health crisis.
For children who receive Exceptional Child Education (ECE) services, an Admissions and Release Committee (ARC) should be convened to discuss whether a change in your child’s placement to Home/Hospital is needed to implement the Individualized Education Program (IEP). The ARC will consider medical documentation you provide. Documentation must be signed by the appropriate licensed medical professional and can include, but is not limited to, the Application for Home/Hospital Instruction, a copy of signed medical records, hospital discharge papers, or a signed statement on office letterhead.
